আমি এই মর্মে অঙ্গীকার করছি যে— ১. এই ফরমে দেওয়া সকল তথ্য সত্য ও সঠিক। কোনো তথ্য ভুল প্রমাণিত হলে কর্তৃপক্ষ আমার ভর্তি বাতিল করতে পারবেন। ২. প্রতিষ্ঠানের সকল নিয়ম-কানুন মেনে চলব এবং নিয়মিত ও সময়মতো ক্লাসে উপস্থিত থাকব। ৩. নির্ধারিত সময়ের মধ্যে কোর্স ফি পরিশোধ করব। ভর্তির পর জমা দেওয়া ফি কোনো অবস্থাতেই ফেরতযোগ্য নয়। ৪. প্রতিষ্ঠানের কম্পিউটার ও অন্যান্য সম্পদের যত্ন নেব; আমার কারণে কোনো ক্ষতি হলে তার দায়ভার আমার। ৫. শৃঙ্খলা ভঙ্গ করলে কর্তৃপক্ষের যেকোনো সিদ্ধান্ত মেনে নিতে বাধ্য থাকব।

I hereby declare that— 1. All information given in this form is true and correct. If any of it proves false, the authority may cancel my admission. 2. I will follow all the rules of the institute and attend classes regularly and on time. 3. I will pay the course fee within the set time. Fees paid after admission are not refundable under any circumstances. 4. I will take care of the institute's computers and other property; I am responsible for any damage I cause. 5. If I break discipline, I will accept any decision of the authority.
